Embryo Donor Checklist

Thank you for considering the donation of your cryopreserved embryos to Embryo Donation International (EDI). You are providing an amazing gift to those in need!

Below is a list of materials that will assist you completing the application. Missing some of the information? You can go back and finish the application later, but everything must be completed before your application will be reviewed. Your work is saved as you go. If you get stuck and need immediate assistance, please call  (01) 239-275-5728 (Monday - Thursday, 9 AM to 4 PM EST and 9 AM to 2 PM on Friday). The application will take you an average of 30-45 minutes to complete.

General Information that you may want to have handy:

  • Where the embryos are stored (i.e. facility name, city, state, phone).
  • The amount of money, if any, owed to the storage facility (i.e., residual storage fees).
  • Embryology data, if available (e.g., Number of eggs retrieved, fertilization rates, day of freezing, number of embryos being donated). Pictures you would like to upload for potential recipients (e.g., photos of your children or childhood/adult photos of those individuals that contributed the eggs/sperm for the creation of the embryos).
  • For each individual who contributed to the creation of the embryos:
    • Their medical, surgical, family genetic history.
    • The most recent sexually transmitted disease test results (e.g., syphillis, hepatitis, HIV, etc.)
    • Any genetic screening test results, if available (e.g., cystic fibrosis, sickle cell disease, etc.)
  • If your embryos were created with donor sperm:
    • The sperm donor profile (includes medical, surgical, genetic screening and social histories).
    • Donor eligibility paperwork (probably in your medical records).
  • If your embryos were created with donor eggs:
    • The egg donor profile (including medical, surgical, genetic screening and social histories).
    • Donor eligibility paperwork (probably in your medical records).
    • If your egg donor had legal representation, a copy of your contract will need to be sent to EDI to review any existing donation stipulations outlined within the contract.

Summary Process:

Below are the basic steps of involved in the donation of your embryos:

  1. Learn the basics regarding the three types of embryo donation procedures available to you.
  2. Complete the application.
  3. EDI will contact you within two weeks of application submission.
  4. Your IVF facility will request their own signed release from you and your partner, if applicable, before they will speak to the EDI staff.
  5. EDI will contact your IVF facility confirming the number and quality of embryos currently cryopreserved.
  6. Some medical information will need to be forwarded to EDI including sexually transmitted disease and genetic screening test results.
  7. EDI reviews all information to determine if your embryos are eligible for donation.
  8. You will need to complete all necessary consent forms.
  9. Your cryostorage storage facility to complete their own transfer paperwork to send the embryos to EDI.
  10. Your donated embryos are received by EDI!
